Saving the data
typed into the enrollment forms requires
Adobe Reader Version 8.0 or higher
so that your form is not blank when
emailed.
You can easily update your version
of Adobe Reader for free by clicking
on the "Get Adobe Reader"
link provided below. This will allow
you to attach completed forms to an
email and send them to the Puppy Lounge
prior to the interview date.
Click
on the graphic to update your version
of Adobe Reader so the information typed
into the enrollment forms can be saved.
Important:
After
completing your forms in Adobe Reader
8.0 or higher, please "save-close-reopen"
the files to confirm that all your information
was properly saved.
Please complete a pet profile for
every pet you wish to enroll. Thanks!
Option
2: Print & Fax or Scan & Email
Please print the enrollment forms after
completing all fields. You are welcome
to scan/email or print/fax the forms
to the Puppy Lounge prior to the interview
date. Please refer to option 1 if you
prefer to save your data and send the
forms as an attachment.
